We have many years experience of failure investigation of Civil Engineering to Offshore Construction. We have a particular expertise in failures related to welded connections.
In addition to our own experienced staff, we have a team of associates we work with in areas such as stress analysis, plant life assessment, materials modeling, welding and corrosion who can supplement our own experience as required.
When it comes to failure analysis, the quality of the examination, the interpretation of the results and the ability to communicate the implications of the results for clients are paramount. Because of this, Arc-Iwe has built up long-term relationships with clients in this area. We act in partnership with their team, feeding back the results of our work as we progress, and agreeing how to proceed based on their knowledge of plant or fabrication.
Arc-IWE is equally at home dealing with the one-off investigation for clients who just want to know what has happened to their material. Much of our work is done direct for clients to help them identify failures in their own materials, plant or fabrications. We also assist in resolving in customer complaints, legal claims and prosecutions, providing an expert witness role.
We have an extensive range of in-house mechanical testing, materials analysis and characterisation equipment available. In addition, we have links to partner laboratories for specialist techniques where we require them to supplement our own methods. We have the workshop facilities to be able to handle large-scale components, and do all sample cutting and preparation in house
Any project can suffer problems, it is part of construction and fabrication. Inevitably these have cost implications and expert guidance is required to control costs. Arc-Iwe are best placed to provide technical guidance as required.
Failure analysis is performed by systematically examining, testing and analyzing the failure, starting with gathering information about the component, its application and history, followed by a detailed visual examination, then nondestructive testing (magnetic particle test, dye penetrant test, low magnification, etc.), leading to high magnification examination and destructive testing (such as: sectioning the sample for Metallographic micro analysis, Hardness testing, Chemical Analysis, Mechanical testing, etc.).

Our Forensic Process
A typical forensic metallurgical failure analysis includes
- Collect failed component background information and service history.
- Perform a visual examination of failed and related components in the as-failed condition.
- Perform low magnification examinations using a stereomicroscope to locate critical areas for further analyses or testing.
- Take dimensional measurements as necessary.
- Analyze the fracture or degraded surfaces utilizing a Scanning Electron Microscope (SEM) at high magnification.
- Using the SEM, determine the failure mode and origin/initiation site. Investigate the initiation site for unusual conditions or contributing factors.
- Identify and characterize the failure mechanism (fatigue, embrittlement, overload, type of corrosion, etc.) Investigate for unusual stress risers.
- Select, prepare, examine and analyze the cross-section microstructure in the region of failure/fracture initiation.
- Analyze conditions of manufacturing reflected in microstructure and investigate potential flawed conditions.
- Investigate material quality and heat treat conditions.
- Perform a chemical composition analysis of the failed component (alloy and impurities), and coatings or platings.
- Perform mechanical and other testing (hardness, strength, toughness, etc.).
- Analyze all gathered evidence and test results to formulate an engineering opinion and conclusion.
- Through a careful extensive forensic examination and evaluation of the evidence gathered from the above analysis, the pieces of the puzzle are fit together in a conclusive manner and a root cause of failure is identified.
All evidence and forensic failure analysis results are photographically documented and presented in a report, which interprets and explains each finding in a clear and concise format.
We also provide: reports of evidence in support of resolving insurance claims, expert witness testimony, and litigation support in product liability and accident cases.
